Section 1. If you want a client to undergo psychological treatment, a psychological assessment must be performed before treatment can commence. This can be an assessment independent of Moving Minds, but must be fairly recent.

Section 2. We will need to have the full company details of the primary referrer, who will be responsible for paying the fees. As Moving Minds subscribes to the Rehabilitation First Code of Best Practice, details of any other parties involved (solicitor or insurer) should be given so that reports can be submitted to both parties simultaneously.

Section 3. The client's contact details will be necessary to arrange the appointment.

Section 4. You will be asked to fill out a brief description of the client's presenting difficulty and the index incident so that we have an idea of the client's needs.

Section 5. If there are any previous reports or medical notes available on the client, it would be beneficial to their rehabilitation if Moving Minds has access to them. If there are any reports included in the referral select the appropriate check box so that we know to expect them. If you are filling out an online referral form you can upload any reports and send it to us via the website.  If you are filling out a printable referral form you can send us previous reports by post or fax.

Once a referral is received you will be sent a letter confirming receipt of your referral and instructions. If you have any queries please do not hesitate to contact us.
